Subacute Care Program Specialized Inpatient Services

Alameda Hospital's Subacute Care Program provides care for patients who require specialized services more intensive than long-term or skilled nursing care, but less so than acute hospital care. Here, patients benefit from the collaboration of an interdisciplinary team. The medical director, registered nurse, respiratory therapist, rehabilitation specialists (PT, OT, and SLP), social worker, dietitian, and activity director work together to develop an individualized plan of care for a wide variety of needs: ventilator dependency, tracheostomies with mist/oxygen, tube feeding, IV therapy, IV med administration, wound care, inhalation therapies, and physical, occupational and speech therapies. Family members are encouraged to play an active role in the plan of their loved one's care and receive frequent progress reports.
